The Clinical Documentation & Coding Coordinator is responsible for facilitating the improvement in the overall quality and completeness of clinical documentation and coding in the Skilled Nursing and Rehabilitation medical records, as well as, responsibility for assisting providers to ensure documentation in the medical record accurately reflects the services rendered by performing assessment of clinical documentation through extensive review of medical records, interaction with physicians, nursing staff, other patient caregivers, and MDS Coordinators to ensure appropriate reimbursement is received for the level of services rendered to patients, and the clinical information utilized in profiling and reporting outcomes is complete and accurate.
